About RxNeuroscience

Psychiatric care at its highest level — without the institutional barriers that usually come with it.

Most patients seeking subspecialty psychiatric care face a frustrating reality: the physicians with the deepest expertise are largely inaccessible in private practice. RxNeuroscience was built to change that.

We offer direct access to two of the most credentialed psychiatrists in the country — delivering care with the time, attention, and clinical depth that complex patients deserve via telehealth in California and Arizona.

Our mission

To make the highest level of psychiatric expertise genuinely accessible.

Patients navigating addiction, aging, ADHD, or complex psychiatric illness deserve more than a brief appointment with a generalist. They deserve a physician who has devoted their career to understanding conditions like theirs.

Our mission is to deliver that level of care — directly, personally, and without the friction of institutional medicine.

Philosophy of care

"Every patient deserves a physician who has the time to understand them, the training to help them, and the intellectual honesty to say when something is complex."

At RxNeuroscience, care is individualized by design. Each appointment is unhurried, each treatment plan built from first principles.

We welcome complexity — patients who have tried multiple treatments without lasting benefit, or who simply want a physician who will think carefully.

What sets us apart

Quadruple board-certified

Certification across psychiatry, addiction, geriatric psychiatry, and obesity medicine — rare in private practice.

Academic faculty & researchers

Faculty positions, fellowship leadership, and peer-reviewed research keep clinical practice at the leading edge.

Unhurried & private

No waiting rooms. No referral networks. A direct relationship with your physician.

Geriatric psychiatry

Yale fellowship · ABPN board-certified

Dr. Joshi is a Yale-trained, ABPN board-certified geriatric psychiatrist and recipient of the APA Hartford-Jeste Award — the field's most distinguished recognition for contributions to geriatric psychiatry. She brings expertise in the full spectrum of late-life psychiatric presentations: Alzheimer's disease and related dementias, neuropsychiatric symptoms of dementia, late-life depression and anxiety, and the complex interplay of psychiatric illness and cognitive decline. She is an active clinical trialist and first-author of multiple peer-reviewed publications on dementia care.

Deprescribing & polypharmacy

Older adults are frequently prescribed multiple medications by multiple providers, creating a complex and sometimes harmful drug burden. Dr. Joshi specializes in careful, evidence-based deprescribing — reducing or discontinuing medications whose risks outweigh their benefits, with attention to cognitive, functional, and quality-of-life outcomes.

Addiction treatment for older adults

Substance use disorders in later life are frequently underrecognized and undertreated. Dr. Joshi combines her board certification in addiction medicine with her geriatric psychiatry subspecialty to offer expert evaluation and management — including alcohol use disorder, opioid use disorder, and prescription medication misuse — with particular attention to the unique pharmacology and medical complexity of aging patients.

ADHD in later life

ADHD is frequently undiagnosed or misdiagnosed in older adults, where symptoms may overlap with cognitive decline, mood disorders, or the effects of polypharmacy. Dr. Joshi offers expert evaluation of ADHD presenting in later life — carefully distinguishing attentional and executive deficits from dementia-related symptoms — and provides individualized treatment with attention to the unique pharmacological considerations of the aging brain.
